NCAA Division I Annual List of Required Forms and Actions

The NCAA has developed a list of required actions and forms that must be completed or submitted on an annual basis by administrators, coaches and student-athletes. This resource was created to consolidate the various actions and forms into a single, central location for use by Division I members. Developed with input from administrators at member schools, this list was compiled by NCAA national office staff in Division I governance, finance, academic and membership affairs, and the Sport Science Institute.

Sports Sponsorship and  Demographic Report Annual reporting of sports sponsored, both for the previous and upcoming academic years. Institutions will provide demographic and contact information for administrators and head coaches. Annual membership dues can also be paid through the system. Report of Uses for Revenue Distribution The NCAA requires that institutions submit expense information pertaining to the Academic Enhancement, Conference Grants and Student Assistance Fund (SAF/SAOF) Revenue Distribution online annually. EADA Report Any coeducational institution of higher education that participates in Title IV, the federal student aid program, and has an intercollegiate athletics program, must comply with the EADA by preparing an annual report, officially called The Report on Athletic Program Participation Rates and Financial Support Data. NCAA Board of Governors Policy on Campus Sexual Violence Annual Attestation The policy, which was adopted by the Board of Governors in 2017, reinforces efforts of the Association in addressing campus sexual violence. By completing the attestation form, an institution attests that it has satisfied the policy requirements. Squad Lists and Instructions Institutions must submit a complete squad list for each sport for student-athletes to represent an institution in intercollegiate athletics competition. Athletically Related Income and Benefits An athletics department staff member who receives athletically related income or benefits from a source outside the institution must report the earnings to the president or chancellor on an annual basis. Drug-Testing Consent Student-athlete must sign form to affirm his or her awareness of the NCAA drug- testing program and agree to allow the NCAA to test in  relation  to any participation in NCAA championships. HIPAA/Buckley Amendment Consent/Waiver Form Signing of the authorization/consent shall be voluntary and is not required by the institution for medical treatment, payment for treatment, enrollment in health plan or for any benefits (if applicable) and is not required for the student-athlete to be eligible to practice or compete.
