NCAA Division I Annual List of Required Forms and Actions
The NCAA has developed a list of required actions and forms that must be completed or submitted on an annual basis by administrators, coaches and student-athletes. This resource was created to consolidate the various actions and forms into a single, central location for use by Division I members. Developed with input from administrators at member schools, this list was compiled by NCAA national office staff in Division I governance, finance, and the Sport Science Institute

| Sports Sponsorship and Demographic Report | Annual reporting of sports sponsored, both for the previous and upcoming academic years. Institutions will provide demographic and contact information for administrators and head coaches. Annual membership dues can also be paid through the system. | NCAA Board of Governors Policy on Campus Sexual Violence Annual Attestation | The policy, which was adopted by the Board of Governors in 2017, reinforces efforts of the Association in addressing campus sexual violence.
