Compliance with Consensus-Based Health, Safety and Performance Guidance
Purpose:Â An active member institution shall provide consensus-based care, education and services to its student-athletes. The institution shall attest annually to the institution's compliance with consensus-based guidance supporting student-athlete health, safety and performance (e.g., Cardiac Care Best Practices, Interassociation Mental Health Best Practices, Concussion Safety Protocol Checklist) as identified, established and maintained through policies and procedures approved by the NCAA Board of Governors.

Division:Â Division I and any Division II/Division III school with a sport(s) that competes in Division I.
Requirements:Â Presidents/chancellors, directors of athletics and campus athletics health care administrators must sign the completed attestation form for each school.
