NCAA Membership Attestation Requirements

The national office has taken steps to streamline the attestation process by providing information on all existing and future attestations on this page. Each policy must be complied with during a given academic year and then attested to in the fall of the following academic year.

Campus Sexual Violence Attestation

Purpose: In 2017, the NCAA Board of Governors adopted an Association-wide policy to reinforce Association efforts regarding sexual violence prevention. 

Due: Nov. 7, 2025

Division: Division I, Division II, Division III.

Requirements: Presidents/chancellors, directors of athletics and campus Title IX coordinators must sign the completed attestation form for each school.

Compliance

Purpose: Each NCAA Division I school or conference attests to compliance requirements for member schools, collaboration with the NCAA when an issue arises, and the importance of creating a culture of compliance on member campuses.

Due: Nov. 7, 2025

Division: Division I and any Division II/Division III school with a sport(s) that competes in Division I.

Requirements: Presidents/chancellors and directors of athletics must sign the completed attestation form for each school.

Legislation: NCAA Bylaws 18.4.2.1.1.2 and 20.2.4.15.

Diversity, Equity and Inclusion Review

Purpose: An active member institution and conference shall complete a diversity, equity and inclusion review at least once every four years and provide written confirmation of completion to the national office.

Due: N/A

Division: Division I. Any Division II/Division III school with a sport(s) that competes in Division I.

Requirements: Presidents/chancellors and directors of athletics for each school must sign the completed attestation form. Conference commissioners and ADIDs must attest to conference requirements.

Legislation: NCAA Bylaws 20.2.4.3 (institutions) and 20.3.4.2 (conferences).

Academic and Support Services

Purpose: An active member institution shall attest annually to providing student-athletes with academic support services consistent with the requirements of Bylaw 16.3.1.1, and career counseling and life skills programming consistent with the requirements of Bylaw 16.3.1.2.

Due: Nov. 7, 2025

Division: Division I and any Division II/Division III school with a sport(s) that competes in Division I.

Requirements: Presidents/chancellors and directors of athletics must sign the completed attestation form for each school.

Legislation: NCAA Bylaw 20.2.4.26.

Compliance with Consensus-Based Health, Safety and Performance Guidance

Purpose: An active member institution shall provide consensus-based care, education and services to its student-athletes. The institution shall attest annually to the institution's compliance with consensus-based guidance supporting student-athlete health, safety and performance (e.g., Cardiac Care Best Practices, Interassociation Mental Health Best Practices, Concussion Safety Protocol Checklist) as identified, established and maintained through policies and procedures approved by the NCAA Board of Governors.

Due: Nov. 7, 2025

Division: Division I and any Division II/Division III school with a sport(s) that competes in Division I.

Requirements: Presidents/chancellors, directors of athletics and campus athletics health care administrators must sign the completed attestation form for each school.

Legislation: NCAA Bylaw 20.2.4.25.

Student-Athlete Career Services (Current and Former)

Purpose: An active member institution shall attest annually to providing student-athletes with academic support services consistent with the requirements of Bylaw 16.3.1.1, and career counseling and life skills programming consistent with the requirements of Bylaw 16.3.1.2.

Due: Nov. 7, 2025

Division: Division I and any Division II/Division III school with a sport(s) that competes in Division I.

Requirements: Presidents/chancellors and directors of athletics must sign the completed attestation form for each school.

Student-Athlete Education: Life Skills

Purpose: An active member institution shall attest annually to providing student-athletes with academic support services consistent with the requirements of Bylaw 16.3.1.1, and career counseling and life skills programming consistent with the requirements of Bylaw 16.3.1.2.

Division: Division I and any Division II /Division III school with a sport(s) that competes in Division I.

Review of Health, Safety and Performance Support Services

Purpose: An active member institution shall complete a comprehensive review of its mental and physical health, safety and performance support services (e.g., legislative and policy requirements, best practice considerations) at least once every four years.

Division: Division I and any Division II /Division III school with a sport(s) that competes in Division I.

Requirements: Presidents/chancellors, directors of athletics and campus athletics health care administrators must sign the completed attestation form for each school.

Legislation: NCAA Bylaw 20.2.4.23.

Coaches Education

Purpose: An active member institution shall provide annual education to all coaching staff members on the career counseling and life skills programming topics listed in Bylaw 16.3.1.2. 

Effective: Aug. 1, 2025, Due: Nov. 6, 2026

Division: Division I and any Division II /Division III school with a sport(s) that competes in Division I.

Requirements: Presidents/chancellors and directors of athletics must sign the completed attestation form for each school.
